The clinical issues of the patient’s end of life care needs were addressed and interventions were made to facilitate hospice services.

This resulted in a cost savings of $12,894.58.

This is taken as a one-time savings and is based on Medicare Cost in hospice cohorts and adjusted for inflation to 2015 dollars.
